The Gender Identity Development Service (GIDS), based at the Tavistock and Portman Trust in North London, was set up initially to provide - for the most part - talking therapies to young people who were questioning their gender identity. But in the last decade GIDS has referred more than a thousand children, some as young as nine years old, for medication to block their puberty. In the same period, the number of referrals has exploded, increasing thirty-fold, while the profile of the patients has changed, from largely pre-pubescent boys to mostly adolescent girls, who are often contending with other difficulties.
Why had the patients changed so dramatically? Were all these distressed young people actually best served by taking puberty blockers and cross-sex hormones?
While some young people appeared to thrive after taking the blocker, many seemed to become worse.
Was there enough clinical evidence to justify such profound medical interventions in the lives of young people who had so much else to contend with?

This is absolute partisan nonsense from a known transphobe trying to kick over stones and pretend they found gold. She’s funded by an international collective of right wing hacks pushing this culture war nonsense and all of this is contingent on the idea that transitioning somehow is not the best medical intervention for people with gender dysphoria. But it is. Time and time and time again, study after study, all around the world, the efficacy is proven to save lives. It is not a cure all but neither is a hip replacement - it’s adequate. This Barnes person is a pure ideologue and this is an embarrassment for you that you posted this with any degree of sincerity. Gross.

Opinion. Perspective. This is another one of those issues that people like to claim has two sides, like they argue the Holocaust has two sides or slavery [the Confederacy] has two sides, and it’s only fair that both sides be given exact weight. This, of course, is nonsense. Nobody becomes Russia by denying people the ability to disseminate their opinion that the Holocaust didn’t happen or was a benefit to mankind; nobody becomes China by suppressing the perspective that we need to return to slavery for the betterment of Black people and acknowledge White superiority. This is mainly because there are always going to be those that actually believe the trash they’re told.
Transgenderism is the same kind of thing. There are not two sides to the “problem”. There are those who suffer from gender dysphoria and those who create laws trying to eradicate transgenderism that don’t. Nobody asks transgender people what laws they’d like to see; when trans kids stand up and speak in public forums about their discrimination and mental health, they get the platitudes “oh, that’s awful, must suck” and the discrimination laws pass unanimously anyway. Imagine how you’d feel if you stood up, told a group of “important” people that what they’re doing would harm you, that receiving this care literally saved your life, just for them to determine your life, and the lives of children like you, aren’t worth saving since you might regret not dying later and they are protecting you from your parents. You can’t imagine that unless it’s happened to you. Arguing about perspectives and opinions tells me it definitely has not.

Strawmen. Hyperbole. Are often used when have when arguments weak and not fully formed. Well…I dunno know but surgically removing perfectly functioning genitalia, or taking puberty blockers to prevent the normal and natural growth of said organs seems a little extreme. Should then people who suffer from xenomelia remove their feet and arms when the standard of care is therapy and the the prescribing of SRI’s not the removal of appendages. No, self respecting surgeon would perform such an amputation, barring amputation or sever deformity, and no sane member of a Medical Board would allow that surgeon to keep his license. Sweden, a very progressive democracy, has already banned gender reassignment surgery and puberty blockers for childern, in except a very few rare cases.I don’t care what adults do, children are impressionable and easily swayed, but should be protected from societal fads.

A terrible medical scandal. That’s exactly right. And so the blame falls on the doctors, not the science nor the patients. Again, it’s like doctors over-prescribing opioids or writing an antibiotic prescription for every runny nose. They may mean well but do a disservice to those affected. Kids that were once getting the treatment they need are grouped together with frivolous treatment and lose it. Considering (at least in the U.S.) that kids have to not only consult with a medical doctor about the treatment and have them sign off on it, they also need to be in therapy (and not just having started therapy) and have a psychologist sign off on it as well. AND they need parental permission. That’s a lot of safeguards in place to make sure that the kids undergoing puberty blockers need to. Nobody is performing surgery on pubescent children because what surgery are they performing? The most common “mutilation” occurring would be top surgery. Nobody needing to start puberty blockers have the required anatomy. Bottom surgery is very rare because it is much more involved and expensive and even most trans adults still have their bottoms even if they’ve altered their tops.
The problem with this book isn’t the research or fair-mindedness; the conclusions will be the only thing cherry-picked and will be used to deny medical care to those who need it. To be fair to those kinds of social crusaders, it is easier to help a child get over their dysphoria when they commit suicide and the problem solves itself. For many who view transgenderism as an abomination, that’s the point.

Would you agree that suicidal depression and suicidal ideation is necessarily multicausal and multidimensional, with a range of therapies being deployed, not merely a single approach?

I think back to a program I watched about people who believe one of their appendages doesn’t belong to them (brought up by cebanks1976 earlier). As they stated, no self-respecting surgeon would perform a surgery to remove an arm or a leg just because someone believed to their core that the offending appendage was a foreign entity. That person is obviously delusional and needs therapy or Jesus or both. It isn’t until they numb the appendage in ice and then begin to SAW OFF THEIR F—ING LIMB that the depth of their conviction becomes manifest. Now no self-respecting surgeon would not try to save their life. Most kids don’t have the wherewithal to mutilate themselves (though some do, and not just cutting). They’ll just kill themselves and be done with it.

Your question is a little tricky, caesar. Is suicidal depression and suicidal ideation multicausal and multidimensional? I would say kinda. Let’s use the example of the very lonely affliction of gender-dysphoria. There are no outward manifestations initially, just that feeling inside. Through socialization and over the years, the feeling gets bigger and bigger. Undeniable. Except through socialization and over the years, they’ve also become more aware of gender expectations and accepted behavior versus unaccepted behavior. They see other kids bullied for being different. But nobody is different like they are. If only there was a book in the library they could read to help them understand what they feel. They have TV and the Internet though. Is it really a sickness like so many demagogues say? Is there something wrong with them? Are they going to Hell for being the way they are? Who can they turn to? Their parents? Hopefully. Hopefully they have parents that love them for who they are and not what’s between their legs. Despite wider education these days about it, sadly, loving a child unconditionally is not sacrosanct and by the time they can give voice to their inner demon, they’ll know whether their family is one they can “come out” to. Do they have friends they can talk to? Do they have friends with enough maturity to empathize with them despite not understanding what they are going through? Maybe they become confident enough to go by a new name, new pronouns. But their teachers all refuse to use their pronouns which gives the students tacit permission to ignore this new identity, too. Maybe they have loving parents but they wouldn’t understand what they’re going through, no matter how many times they try explaining it. They can’t talk to their teachers because state law says they can’t. They are alone in a world that everyday reminds them that it hates them, wants them eradicated. They see people like them standing up, telling those in charge what they are doing is harmful to them. They watch those in charge pass those laws anyway. What’s the point of it if all they can look forward to is a miserable life being hated for who they are? There is no point. It’ll never get better. Why did they have to be like this? There’s only one way out for them.

So, yes. They didn’t get to suicide because of one thing. It was a number of things. But it wasn’t a number of widely disparate things. It all stemmed from a central root. My example is single-minded [toward a predetermined outcome] and simplistic. Lots of trans kids become adults because of the support system they had growing up, parents, doctors, therapists, friends. Lots of trans kids escape the horror of their childhood and flourish as adults without that support. Lots of trans kids aren’t “strong” enough to give adulthood a chance. And some trans adults circle right back and give up the fight. Because it is a fight, every day, just for them to live their lives, a war you and I couldn’t fathom because we were born “normal”. I’m not saying we need to empower every child to make adult decisions. What anti-trans laws do, though, is empower the wrong adults to make decisions other adults should make, like parents and doctors and not campaigning politicians.

The vital importance of compassion & real empathy for people who do have the condition of gender dysphoria (compassion for all people, ideally) is indeed absolutely necessary. Virtually all parents - if not all - want the very best for their children - for every child. And politicians are parents too. I’m not particularly fond of politicians, but I don’t want to slip into the obvious error of dehumanising any group or category of person.
What I said was not meant to be tricky. The reality of a complex condition like suicidal depression with suicidal ideation is that it is necessarily multicausal and multidimensional, with a range of therapies being necessarily utilised, not merely a single - and the most radical - approach.
Otherwise, we fall into the trap of reductionism.
For a mature adult, it can actually take years of counselling to unlock the complex, essential causes of their plight - if indeed they ever make such a breakthrough. There is no magic bullet - certainly not in the case of the young, developing mind of a child. To use that phrase, the problem is often not about what it’s about. There are many issues & dimensions folded into each other, and not even clear to the person themselves.
There is too much of a focus on body image in society. The media/net can be poisonous in this regard. As has been said, children can be exposed to stuff which can have a deeply negative, formative impact on their young, developing minds. There seems also to be a growing issue now of litigation having a chilling effect on counsellors ability to offer the full range of necessary, therapeutic services that were once available. This, needless to say, is incredibly destructive.
For an adult, the freedom to make choices with regard to the course they wish to take - even involving radical surgery - is, I believe, absolutely necessary in a liberal, democratic society. Of course, long-term counselling & consultation is necessary, and this has been the norm in the medical/legal sphere for many years now. It’s their decision as a fully grown adult.
In the case of a child, who has not yet reached maturity, whose rational faculties have not formed, and who cannot render fully informed consent, due to the reality of their being a minor - underage, a child - adults making decisions to inflict radical, surgical interventions upon that child’s young body - such as the removal of breast buds, as mentioned - is wholly reprehensible.

Irreversible medical procedures (which involve surgical interventions that are permanent) as a “treatment” for mental illness?
Lobotomies were all the rage for treating mental illnesses in the 1930s - 1960s. We look at that with horror now, as people in the future will view the race to “transition” these poor suffering individuals.

All the detractors of this book have is insults and name-calling. They have nothing else. Their ideology is incoherent, anti-science, anti-biology and pseudoscientific. The idea that people can self-diagnose their condition, be told their distressed feelings are correct, then remake the body to reinforce the distress, rather than the other way around.
It’s noteworthy that there is no call for a cure for this distress; even the suggestion is reframed with the hilariously dishonest term “genocide.”
It’s also notable that all the ideologically-captured institutions such as Stonewall and HRC have no only embraced an overt form of homophobia by redefining “gay” to be “same-’gender’ attraction” (where ‘gender’ is poorly and/or circularly defined), but they’ve also eliminated the require for dyphoria from their definition of “trans.” That’s because this has nothing to do with that. This is a form of gnostic religious cult in which the point is to untether from the limits of biology (i.e. reality). Remember all those cults where they talked about transcending their mortal bodies, and then killed themselves to do it? It’s like that. That the human body is just a vessel (it’s not) and that people should be able to remake themselves according to their fantasies, rather than being bounded by reality. That’s what the accusations of “hate” are about: being constantly jolted back to reality people who decline to participate in and endorse their delusion.
You are not against trans people by opposing this cult; there are many trans people who are fighting this insanity. And the reality is that the overwhelming majority of the people pushing this junk and calling you “transphobic” aren’t trans themselves. They often call themselves “nonbinary” but that’s like “queer” - a way for unremarkable people to feel special. “Queer” is straight people pretending they’re almost-gay. “Nonbinary” is normal people pretending they’re trans-adjacent.
Specifically regarding this book, it’s clear from the detractors that not a single one of them has actually read or listened to it.

If they had, they could point out the flaws. They can’t. Because a) they’ve never read it and never will (heresy and blasphemy), and b) like any cult, they take the position that if you don’t believe it then you don’t understand it. If you understood it, then you would believe it. They don’t, and can’t, acknowledge that you can understand it and not believe it or endorse it. So, they have to lie and pretend.
